Oklahoma Kids Performers Contract Come join the fun and show off your talents - Join OK Kids today, fill out the Member Application and the Performers Contract and mail to the address at the bottom of the Member Application!
Oklahoma Kids Group Competition Form Group Competition form should be used when two or more performers will be performing the act.!
Oklahoma Kids Solo Competition Form Solo Competition form should be used when one performer will be performing the act.!
A PDF document will be downloaded and displayed for printing. Forms can be mailed to address at bottom of form or Faxed to (918) 512-6754. Faxed forms will be processed via Paypal. A Paypal invoice will be sent to the email address listed on the form. Please do NOT include credit card information on entry forms
